Disciple Maker Index (DMI) survey

Dear Philadelphia Pastors and Parish Leaders,

With only a few days left before the launch of the Disciple Maker Index (DMI) survey, we want to thank you for all the hard work that you have put into setting this up for success!

Please announce at Mass:

The survey will be open beginning February 16th. There are two ways your parishioners can take the survey:

1. Online. www.DiscipleMakerIndex.com
   Post this link on your Website, social media sites, and include it in your bulletin for the duration of the survey (February 16 th to March 18 th).
   If your bulletin for the upcoming weekend has already been printed, we would encourage you to print a bulletin insert or separate handout for the pews with the link and instructions to access the survey.
   Use your parish email lists/database to email your parishioners this link with a request to take the survey.

2. Paper Survey.
   You should have received 100 paper surveys by last week. PLEASE CONTACT ME IF YOU HAVE NOT RECEIVED PAPER SURVEYS.
   Feel free to print additional paper copies of the survey using the attached PDFs.
   Please have a place available for parishioners to drop off their completed surveys. I will provide you with FedEx return mailing labels for all completed paper surveys.
   All completed paper surveys must be received by Catholic Leadership Institute by March 22nd at the very latest to allow for data entry.

Consider how you will publicize the survey once it opens:

Here are some opportunities:

• Email Lists/Parish Database: Continue to email your parishioners the link with a request to participate.
• Mass Announcements: Speak about this opportunity to parishioners at all Masses throughout the week.
• After Mass: Consider having tables with paper surveys and laptops available after Mass to catch people on their way out.
• Parish events: Consider any other parish events happening while the survey is open. Make a pitch or ask a ministry leader to remind parishioners then.
• Bulletin: Print the link www.DiscipleMakerIndex.com in the bulletin
• Website/Social Media: add the link to your website and any other social media that you have.
